Esperanza Health & Wellness Center is a private 501(c)(3) non-profit organization whose principal mission is to provide individuals and families with compassionate and comprehensive care to promote wellness and build strong communities.
MISSION
Esperanza Health and Wellness Center’s mission is to meet the immediate needs of all individuals and families we work with and strengthen their capacity to be well and thrive.
EHWC’s programs and services help meet the immediate crisis needs of individuals and families, and support them in developing skills, and connections to resources when needed to address the challenges that have kept them from thriving and succeeding.
WHAT WE DO
EHWC helps individuals access positive and healthy social, physical, and economic environments that promote attaining the full potential for health and well-being for all. Esperanza Health and Wellness Center is a bridge for “Social Determinants of Health and Social Determinants of Life” (SDOH and SDOL).
Social determinants of health (SDOH and SDOL) have a major impact on an individual’s health,
well-being, and quality of life.
Examples of SDOH include:

Access to appropriate health services

Safe housing, transportation, and neighborhoods

Racism, discrimination, and violence

Education, job opportunities, and income

Access to nutritious foods and physical activity opportunities

Polluted air and water

Language and literacy skills

Financial literacy skills
(SDOH and SDOL) also contribute to wide health disparities and inequities.
Example:
Individuals who don't have access to grocery stores with healthy foods are less likely to have good nutrition. That raises their risk of health conditions like heart disease, diabetes, and obesity — and even lowers life expectancy relative to people who do have access to healthy foods.
Just promoting healthy choices won't eliminate these and other health disparities. Instead, EHWC and our partners in sectors like education, transportation, and housing are taking action to improve the conditions in people's environments.
